Very creative! Though really, the way it is written I can't really fault you on your format, ideas, or word choice. ( : / ... Now what am I supposed to say.) You had a nice choice of key words to convey the thought of the poem while making it so that it flows well.
Some Ideas (Because as I said before, nitpicks seem a little out of the question on this poem):
1. (This is a picture, right? I meant to compliment you on the font style up above in any case. I don't know...I thought it fit well with the poem. Anyway...) If you could get a picture of even some of the actual quiz you could cross out in Sharpie all the words you didn't use, and it would look cool.

Pretty much all I can say is that this is awesome. I would never have thought of this! The creativity is amazing and it makes for an interesting poem. I enjoy that fact that you kept the set-up the same.
I feel that the odd spaces add instead of detract from this piece. If I don't stop now, I could probably keep complimenting this all day.
