This is a really nice little piece here. I like how the whole poem is a metaphor. Its pretty straightforward and doesn't go off the tracks so good job there too.

My favourite stanza is the second one. How you personified the heart was really nicely done.
"I found my heart blue and shaking,
It nearly suffocated the last time,
Cowering, cold and small in the corner of my ribcage."
Gorgeous lines.
I don't have much criticism, but maybe the word 'key' less and replace it with some synonyms because it gets kind of repetitive.
