Instruments of pain.
They mold perfectly into your hand,
play a melody of soothing heat,
that cuts and burns into your skin.
You know you should stop,
but you crave the feeling.
You need that soothing releases,
as heated metal meets skin.
Lighter and screw driver,
the objects of your destruction.
They tell you everything is going to be okay,
when no one else will.
They cradle you to sleep,
on the nights you break down and cry.
You find the more you burn and scar you self,
the more you depend on the feeling to feel relaxed.
You feel like you’re digging yourself into a hole of relents and pain.
After a while you hope something or someone will save you.
That something will chase away the demons.

This was really deep, and it captures perfectly how some people crave that feeling, that need to do self harm. Its very similar to an addiction, you just cant stop, no matter how hard you try, you still crave the pain and the emotional relief you get from it.
I love the lines:
After a while you hope something will save you.
That something will chase away the demons.

You write with so much emotion, it seems you just pour your heart and soul into it.
This is similar to so many peoples real life situation.
Them finding comfort with the blade and so on.
My favorite line is:

They kind of show how some people NEED the blade, the lighter, the pain.
It comforts them when no one else is there to do it for them.
I also like that you don't have a rhyming scheme.
You can write really well in that style.

I enjoyed your poem. I love the metaphor of the weapons being "instruments" that "play a melody", it's really clever.
But there is one major problem I noticed. You sound wishy-washy.
Throughout the poem, it seems as though you're trying to say that self harm is the only thing that will make the character feel better. But at the end, you turn it around in a way that's a bit jarring. You say that the character wishes there was a better way to make him/herself happy.
I feel like the ending would be so much more powerful if you hinted at this throughout the poem. Hint to the audience that the "instruments" aren't the perfect remedy, make the change of tone gradual, and then at the end turn it around and say that you want something that will "have you playing a different song".
I also don't like how you tell us exactly what weapons you're using. Seems unnecessary.
Overall good poem, the line breaks are great and the tone is nice.

Sigh....this brings back many horrible memories of my past. But before I go into that, let me just say that this piece is simple and clear-cut but pretty wholesome. It describes quite accurately the struggles of a self-harmer. I used to cut...was on meds for depression after developing an ED. But you know what? 3 months ago, the Lord set me free. I literally saw my demons, and they left. Now idk about your personal beliefs, but I just want you to know that as long as there's breath in your lungs, there is hope. I hope that you'll soon be playing a different song. <3

This is a really good poem on self harm. I'm familiar with the subject - not in a person, experience way, but one of my friends used to struggle with it. This reminds me of what she used to think.
