And you laughed at all my jokes,

First off, I think you can do without the beginning.

Everything from there up, I don't think it's necessary, and it ruins the flow of the piece.
Now. Onto everything after that intro. I love the way you repeated the name "Matt" because it really made the poem personal.
This is so well written, so sad, so happy. It's amazing and If someone wrote this for me I'd be crying. Beautiful piece. Well done.
