is it up to the rape victim to report the rape or is it the specialists at the hospital job?

the victim. They can give a description of the aggressor, but the doctor can only prove whether one was raped or not.

The victim is the one that can press charges, though the doctors and nurses and really, anybody who wants to help the victim, can do their best to make sure the victim gets represented.

Both. It's actually basically anyones obligation to report it, including doctors of any kind. Rape and abuse are two of the things that people have to report no matter what. But the victim by law does not ever have to press charges, that is their choice.
