The tear drop falling down the cheek with a smiley, it's an amazing idea to emphasize that tears do show it all. And the idea of the title itself could be really powerful if you decide to write a song about it, or maybe a simple poem. If you do that, I'd truly love to read it. You did a great job!

I'd like to start out that the picture is nice, and I like the idea of the picture. However, I really think you executed it better. I suppose the main thing is that the idea of tears showing your sad is rather obvious. And most people can tell when one is sad by the facial expression first. To really make the idea come to life is to have an expression or colors that contradict what the tear means. Your face is already solemn, and the picture is in black and white, which tends to hint towards sadness. A world void of color tends to be depressing. So the tear isn't even needed to exactly show that she's sad. We can already guess.
So my opinion is, in order to really get the idea across that tears "Show it all" is to have them contradict the atmosphere and the facial expression. Of course, that's just my opinion.
Hope this helped in some type of way, whether to give you more inspiration for another piece or just help you to withstand criticism on your art. Which I always hate, but always take it to heart ;p
