I liked it, but I do think that the single rhyme in the last stanza really interupted the flow. I think if you changed the poem so it didn't rhyme (which is a lot easier than the other way around!) you could really help it.
This is a good poem, and as I read it, I got sucked in.

The only thing I felt was a jolt in the last stanza, but I can't pinpoint what it was. I am thinking perhaps an unseen rhythm was broken because it seemed slightly irregular. But I'm just nitpicking.
