Italics with Italics

Okay, so I've reached a part in my book where there's a story being told, or, rather, an event being relived. The whole passage for this section is in italics. But I'm having trouble - what do I now use as italics? If it's all italics, and italics are needed for a particular instance, what's a good way of doing that? Should I just have it like this:

The whistling choo! of the train as it approached

Or some other way? Please help!

When everything is in italics, then emphasis via removing them as you've shown is the commonly accepted method. Technically you could also go for bolding or underlining, but I strongly recommend you not use those as they tend to feel a bit too much.
