I did this program in 6th grade where my English was writing a "novel" (wasn't that long-only 40 pages or so). Anyeays they had this questionnaire thing a lot like this, in fact almost exactly like it. I still use some of the tips in there too. I love these things, but you're right Snoink-can't get too caught up in making sure characters are picture-perfect.

If you haven't developed your character should you use this or if you are in a writer's block that has to do with your character? Did I explain that alright?

Depends. If you like to outline/know everything before you start a story, then fill it out then. If you have a character-based writer's block, filling out a sheet like this is often helpful. But as several people have said, don't stick to what you've written down ad-verbatum. It's a starting point, and if your characters change, they change!
