How do you come up with Land names?

I pick a letter I like and try to think of something fantasy sounding.

It probably be best to change a letter or two then, just to be sure you don't use somebody's name. -
yeah. i usualy choose names from a different ethic group like japanese or welsh.

I usually just make something up that sounds interesting, and has the connotation I want. For the planet that my characters are on, I actually took one of my friend's last names and added on to the end. Everything else is generally totally random.

I think that I'm actually going to change a word that is translated from "power" or something. I don't really care, I just need a cool name that works

I used a name i made up years ago and shortened it. from Rensue to Ren. so now the continent my story takes place in is a land called "Ren."

How does a land get a name? It's actually a really good place to start your region-building.
Maybe it was first discovered by raiders (the world's first explorers). Did they find it comparatively snowy, hot, rainy, barren, warlike, peaceful than their own land? If so they probably gave it a term in their own language to denote that first impression.
For an entire world, the first ones who thought in that scale would be the religious (probably). Merchants and scribes probably don't have a need for a term for the entire world, especially if it hasnt been completely discovered yet. If you're building a religious/spiritual culture, crib something from that.
