I've been called far too many things for my liking. 'Cabbage' was one of the consequences of blue hair dye and bleach both fading and becoming an odd shade of green. 'Plimsole' was derived from it sounding like my original last name but died off when the creator left school.
The main (and obvious) one I get called is Beth. Though, I've been called it against my will for so long, that calling me Bethany now sounds foreign. Even so, I still don't like being called Beth
I think it turned ten o clock but I don't really know
And I can't remember caring for an hour or so
Started crying and I couldn't really stop myself
Started running but there's no where to run to

Insulting nick names (offline or online my personality inspires them):
Freak
Weirdo
Insane
smart ***
Small (a joke on my last name that happens in real world only, no small is not my last name)

Online: Zol or anything they could rhyme with Zolen.
Offline: I have no nick name other the ones meant to insult.
Self quoting is the key to sounding wise and all knowing.

I've been called M.T head because of my initials. (Pronounced "empty.") I've been called an idiot, weird, crazy, specs, four-eyes and all the typical insulting names. My best friend knighted me as Sir Stick-hair when we were seven, but nobody calls me that anymore. Online, people usually call me Pom or Pomp, though I don't mind Pompsicle much.

On YWS I get called Tri sometimes, but I don't really have any nicknames. On my 16th my friend decided she was going to start calling me Shazza, but never actually started using that as my nickname.
Growing up I got Sarbear and Sarahsaurus (more dinosaurs) and some of the boys in my class called me Seznevs for a while - which was super annoying.
I did have one epic nickname when I was in grade one, when one of my friends called me the Fancy Guinea Pig (there was a back story to this xp)

Offline I am The Man With The Hat, or more commonly among my enemies Briefcase W****r.
And to @ConverseFireGirl I am Bubblemuncher for no particular reason

Let's see. Offline, I've been called:
Noodle (by my parents, no idea why)
Hagrid (because of my hair)
Tucanette (because it sounds like my name)
Online, if people don't refer to me with my full username, it's either clevs or clogs.

Online, basically everyone calls me either just fortis or fort, or sometimes forty or forties (which I don't like as much). It's pretty universal, really.
Every now and then someone comes up with something different for me,
Solitude, Fort Knox, ph0rtis (or derivatives), K, 40's...

Offline people just call me by my name. That's also rather universal. There's hardly any nicknames. My mom calls me "big baby" though. XD
Instead, he said, Brother! I know your hunger.

Online I'm pretty much universally called Dragon. Knight Dragon's YWS specific, but still not as common as Dragon. And apparently I caused a lot of confusion among users (especially the mods) who thought my username was dragonofphoenix, so I don't know if that counts. XD

Offline, I've been through the whole derivations of my normal name. My favorite nickname, though, is "Microwave." A friend's younger sister refused to say my name and called me that instead (even though she was fully capable).
